(2) “If x =2z then y = 3z, and if x =2v then y = 3g.”
(3) “If x =2z, then y = 3z and if x =2v, then y = 3g.”
(4) “If x =2z, then y = 3z, and if x =2v, then y = 3g.”

I believe the answer is (3). If/then statements require commas. However, the two statements form a list, making a comma before “and” unnecessary.
